EH&S Background Data / Define Incidents

The Environmental Health & Safety application allows you to track safety-related incidents to ensure that adequate follow-up and remediation occur. The table below illustrates the background data associated with incidents.

Incident Data Item Examples
Root Cause Category

Administrative failure, engineering failure, unknown, etc.

Incident Types

Accident-Reportable, Accident-non-reportable, Other

Short-Term Corrective Actions

Disable equipment, discipline employee, close area, etc

Long-Term Corrective Actions

Modify equipment, modify procedures, require training prior to use, etc.

Injury Categories

Burn, chemical burn, laceration, sprain, fracture, etc.

Injury Areas

Abdomen, arm, back, chest, face, ear, leg, etc.

Define Incident Data

To define incident root cause categories:

  1. In the Process Navigator click EH&S Background Data to open the background data entry tab.
  2. Under Incidents click Define Root Cause Categories.

    Previously defined categories are listed in the middle pane.

  3. In the middle pane click Add New to open the entry form in the right pane.
  4. In the entry form enter a root cause category code and optional description for the category.

    A category code might be Administrative Failure, Engineering Failure, etc.

  5. Click Save at the top of the input form to save the item to the database.

To define incident types:

  1. In the Process Navigator click EH&S Background Data to open the background data entry tab.
  2. Under Incidents click Define Incident Types.

    Previously defined types are listed in the middle pane.

  3. In the middle pane click Add New to open the entry form in the right pane.
  4. In the entry form enter an incident type and optional description.

    A incident type might be Accident-Reportable, Accident-Non-reportable, etc.

  5. Click Save at the top of the input form to save the item to the database.

Define Corrective Actions

To define short-term corrective actions:

  1. In the Process Navigator click EH&S Background Data to open the background data entry tab.
  2. Under Incidents click Define Short-Term Corrective Actions.

    Previously defined actions are listed in the middle pane.

  3. In the middle pane click Add New to open the entry form in the right pane.
  4. In the entry form enter a short-term action and optional description for the action.

    An action might be Disable Equipment, Close Area, etc.

  5. Click Save at the top of the input form to save the item to the database.

To define long-term corrective actions:

  1. In the Process Navigator click EH&S Background Data to open the background data entry tab.
  2. Under Incidents click Define Long-term Corrective Actions.

    Previously defined actions are listed in the middle pane.

  3. In the middle pane click Add New to open the entry form in the right pane.
  4. In the entry form enter a long-term action and optional description for the action.

    An action might be Disable Equipment, Close Area, Require Training, etc.

  5. Click Save at the top of the input form to save the item to the database.

Define Injury Information

To define injury categories:

  1. In the Process Navigator click EH&S Background Data to open the background data entry tab.
  2. Under Incidents click Define Injury Categories.

    Previously defined categories are listed in the middle pane.

  3. In the middle pane click Add New to open the entry form in the right pane.
  4. In the entry form enter an injury category and optional description.

    An injury category might be burn, laceration, fracture, etc.

  5. Click Save at the top of the input form to save the item to the database.

To define injury areas:

  1. In the Process Navigator click EH&S Background Data to open the background data entry tab.
  2. Under Incidents click Define Injury Areas.

    Previously defined areas are listed in the middle pane.

  3. In the middle pane click Add New to open the entry form in the right pane.
  4. In the entry form enter an injury area and optional description.

    An injury area might be abdomen, arm, back, chest, head ,leg etc.

  5. Click Save at the top of the input form to save the item to the database.
