Add a Header to a Questionnaire

Headers play an important role in organizing your questionnaire and making it easy for users to navigate and use. Headers can take many forms:

For example, the Fire-LIfe Safety Survey in the sample HQ project uses several headers. The title at the top of the questionnaire, Fire-Life Safety Survey, is defined in the questionnaire profile; all text in blue blocks are headers.

Headers are stored in the Questions (question_ext) table and are added in the same manner as adding a question. The value for their Question Type field is "Label." You create, save, and set the display order for headers in the manner you do for questions. For information, see Define Questions.

To review or edit a header, select a header from the Question List and its values display in the right pane. For example, the below image shows the values for the Instructions header in the above questionnaire. You can also search for an existing header using the Search Questions tab on the left. When you find the desired header, click on it, and you can review its values in a pop-up form.

There are a few ways to create a new header record. See Define Questions: Creating New Questions and Headers. The following fields in the Questions (question_ext) table define a header.

Display Order The system sets this option to reflect where the header is placed in the Question List. You can reposition the header. See Setting the Display Order

Prefix

An optional short string

Text Enter the text to appear in the header.
Tag A short identifier for grouping related headers. It is useful to enter a tag for a related group of items so that you can easily search for them in the Search Questions pane. For example with the HQ sample project, if you enter "fire" in the Tag option of the Smart Search Console of the Search Questions pane, you can see a list of all questions and headers that pertain to fire inspection surveys.
ID The system completes this field to uniquely identify the question.

A Header with Answer Options

As you examine the sample questionnaires in the sample HQ project, you will notice that the header often contains the labels for the responses. For example, the below header contains the rating choices:

You can have descriptive text for the answer options, as shown below.

To create headers such as this, in the Answer Options section, choose Add and the console presents a pop-up form for entering the answer text and sort order. Use the Sort Order to set the order, left to right, in which the answer options appear.

If your survey has many questions for the same header, the user may scroll past the header and need reminders of the answer titles for each column. In this case, you can include the answer titles directly in the question. See Multiple-choice Questions.
